How to use custom templates

Customize note structure, headings, formatting, and specialty workflows in Skriber.

Templates control the structure and style of your generated notes. A good template helps Skriber create notes that match your specialty, EHR, and preferred documentation style.

What templates can control

  • Note type, such as SOAP, H&P, therapy note, psych follow-up, or intake.

  • Section headings and section order.

  • Bullet vs paragraph style.

  • Details that should always be included or omitted.

  • Specialty-specific phrasing and documentation expectations.

How to request a custom template

Contact support with the following information:

  • Your specialty.

  • The note type you want.

  • The exact section headings and order.

  • Any sections you do not want.

  • Whether you prefer bullets, paragraphs, or a mix.

  • A de-identified example of the format you want.

Do not send unnecessary patient details

If you share an example note, remove patient-identifying information unless support has given you a secure process for that specific request.

After your template is updated

New visits should use the updated template. For an existing visit, regenerate the note after selecting the updated template so the new structure can be applied.

If formatting still looks wrong

Send support the note section that is wrong, the desired output, and whether the issue happens on one visit or every visit using that template.
