Review a visit transcript alongside your note

Compare the available transcript with your note and identify whether a missing detail is a source or note issue.

The transcript is the text captured from a recording. The note organizes visit information into your selected template. Comparing them can help you locate a missing detail before you correct the note.

Compare the visit sources

When a transcript is available for the intended visit, review the relevant passage alongside the note. Check whether the detail appears in the recorded conversation, the note, or any Visit Context you supplied.

Verify names, numbers, and clinical details against the original encounter information. A transcript can mishear speech or omit content, so it is a reference for review rather than a final clinical record.

What the comparison tells you

  • The detail is in the transcript but not the note: review the selected template, Visit Context, and Custom Instructions, then make the appropriate note correction.

  • The detail is absent or wrong in the transcript: do not expect a note rewrite to recover speech that was not captured. Correct verified facts in the note and ask support if the recording or transcription appears incomplete.

  • No transcript is available: check the visit’s processing state and follow My note is stuck or missing. Do not delete the visit or reset the recording device while recovery is being checked.

For an exact factual correction, use Edit your note manually. Review the full corrected note before using it in the EHR.
