Fix missing or incorrect details in a completed note

Check the visit sources, correct a completed note, and know when to contact support.

Use this guide when a note has finished generating but a detail is missing, incorrect, or in the wrong section. If the visit is still uploading or processing, start with My note is stuck or missing.

Check the visit and its sources

  1. Confirm you opened the intended visit.

  2. Compare the note with the available transcript and any Visit Context you supplied.

  3. Check which template was selected. A different template can organize the same visit differently.

A transcript can contain errors or omit speech. Use your clinical knowledge and the original visit information to verify a fact; do not treat either the transcript or generated note as the final authority.

Choose a focused correction

  • One factual detail is wrong: use manual editing to correct the note. Confirm it saved and review the surrounding text.

  • A section needs clearer wording: use Magic Edit with a specific request, then check every fact in the result.

  • The organization or recurring style is wrong: review the template and Custom Instructions rather than repeatedly fixing the same preference by hand.

If you change the visit sources or settings and decide to regenerate, preserve the current note first and follow Regenerate a note. Regeneration changes wording; it cannot restore audio that never uploaded.

If a section or expected heading is missing

Check the selected template. If the result still does not match the expected structure, contact support with the template name and a de-identified example of the missing heading or section. Do not assume repeated regeneration will fix the issue.

Before charting

Review the complete corrected note before copying, downloading, or syncing it. If an earlier version is already in your EHR, check and correct that copy separately through your organization’s workflow.

Open the Help Center support messenger. Include your account email, device or browser, approximate visit time, template name, and a brief description of what was expected versus what appeared. Avoid patient-identifying details in screenshots or examples.
