Correct a note after moving it to your EHR

Review the Skriber revision and the EHR copy separately, and check the workflow before transferring again.

If you discover an error after copying, downloading, or syncing a note, review both the Skriber note and the version in the EHR. Do not assume that a later Skriber edit has changed the charted copy.

Correct and review the Skriber note

  1. Open the intended visit and compare it with the EHR encounter.

  2. Make the verified correction using manual editing or the appropriate note tool.

  3. Confirm the change saved, then review the complete note.

Changing a template or regenerating a note can change more than the original error. Recheck the entire result before transferring it.

If you used Copy Note or a section copy

Open the correct patient and encounter in the EHR and follow your organization’s correction workflow. If the EHR note is still editable, update the affected text in the intended field and review the result. If you copy the revised text again, check for duplicated paragraphs, headings, and conflicting versions.

If you attached a PDF

Review the existing attachment and follow your organization’s process for correcting or replacing it. Generate a new PDF from the reviewed Skriber note if that process requires one. Confirm the corrected document is associated with the intended patient and encounter.

If you used EHR Sync

Before syncing again, confirm the behavior for your supported EHR setup with Skriber support or your organization’s administrator. This guide does not establish whether a repeat sync replaces, appends to, or creates chart content. Review the destination fields after any transfer.

If the EHR note is signed or locked

Use the EHR’s correction or addendum process under your organization’s policy. Contact the person responsible for that workflow if you are unsure how to proceed.

Final check

Verify the patient, encounter, factual correction, section placement, and final wording inside the EHR. Keep any remaining prior version or attachment only as required by your organization’s workflow.
